Four County Counseling Center CONTINUUM OF CARE

EMERGENCY SERVICES: (All Ages) Four County Counseling Center provides 24-hour crisis intervention services. An assessment of the consumer is made by professionals and entry to our Acute Care Unit may occur if appropriate, or an Intake appointment will be made for a more appropriate service environment. ACUTE CARE SERVICES: (Primarily Adult) Four County Counseling Center has a 15-bed Acute Care Unit located in Logansport, Indiana. It provides a 24-hour a day inpatient care for consumers who are experiencing acute psychiatric distress, severe crisis, who are in need of medical adjustment that cannot be accomplished on an outpatient setting, or those clients who are court-ordered to Four County Counseling Center for inpatient settings. A Treatment Team, consisting of a psychiatrist, nurses, and other clinicians, as well as many mental health workers, assists each consumer in developing an individualized plan of care. The average length of stay for the Acute Care Unit is 3-5 days. The treatment team reviews the Treatment Plan of care and determines when the consumer is ready to be released from the Acute Care Unit. The consumer may be released to the Group Home and access Day Treatment services. RESIDENTIAL SERVICES: (Adult) Residential Services are provided by Four County Counseling Center through the Residential Program and the Semi-Independent Living Program. Four County Counseling Center has two group homes with a total of 25 beds, one of which is kept for a respite bed for those consumers who need a brief time away from their current environment, but are not in need of inpatient treatment. Our group home staff assists our consumers in learning how to live more independently. While residing at the group home, many of our consumers participate in Day Treatment as well as other services. Our Alternative Family for Adult Program offers our consumers an opportunity to live in a family setting within the community. The family environment provides natural supports in addition to the care management provided by the staff. After successful completion in a group home setting, the case manager, the psychiatrist, and the consumer determine the need for a lesser restrictive setting. The consumer may be considered for the Semi-Independent Living Program where consumers have minimal supervision by staff. DAY TREATMENT THERAPY: (all ages) Four County Counseling Center provides Day Treatment Therapy, which provides a structured environment for those consumers who do not need 24-hour care, but who are experiencing moderate to chronic psychiatric or emotional disabilities. These consumers are able to remain in the community while obtaining the needed intensity of support provided by the Day Treatment Program. This treatment offers a combination of individual and group therapy. Transportation is provided for those consumers who are in the satellite areas and participate in the Day Treatment Program. There is an Adult Program and an Child & Adolescent Program. From this program, a consumer may advance to the Community Support Program. COMMUNITY SUPPORT PROGRAM: (Adults) The community support is designed to enable the chronically mentally ill consumer to successfully integrate into the community. The staff provides case management, Clubhouse services and individual therapies. These consumers may live independently in the community or live in supported independent living apartment. The consumer, in conjunction with the treatment team, transitions into Outpatient therapy if appropriate, to work on individual issues. OUTPATIENT SERVICES: (all ages) Outpatient Services is a service provides treatment for people of all ages and backgrounds. Outpatient Services may include individual therapy, group therapy and/or family therapy. Consumers may also access Medication Clinic for medication monitoring. When the consumer along with the therapist and psychiatrist determines that there is no longer a need for Outpatient services, the consumer may then transition into our least restrictive service area of medication monitoring. MEDICATION CLINIC: (All ages) Registered nurses and the psychiatrist monitor medications that are prescribed by Four County Counseling Center’s psychiatrist. Medication Clinic only is the least restrictive service area on our continuum of care. Other Four County Counseling Center consumers also access medication clinic while attending various service areas. BRIDGES ENTERPRISES: (Adults) Bridges Enterprises provides supported employment services and job placement for consumers with psychiatric disabilities. The staff provides support to the consumer and to the business to help successfully establish the consumer in the workplace. The consumer may attend CSP services in addition to Bridges services. ADDICTIONS PROGRAM: (All ages) Addictions treatment is an integral part of Four County Counseling Center. We are one of the few centers in the State of Indiana, that is an approved Treatment Center for gambling addictions, as well as substance abuse addictions. Consumers that have an addiction illness may access those services through several different programs. Inpatient detoxification services are provided by contractual agreement with the ASPIN network. When the consumer no longer needs inpatient care, the treatment team determines the level of care for services within Four County Counseling Center. We offer a 4-week alcohol/drug therapy program, a 6-week Intensive Outpatient Program, and a 12-week Intensive Outpatient Saturday program as well as a 4-week aftercare program. Our adolescents have programs that are similar to the adult programs; however, they are geared for adolescents only. HOME BASED SERVICES FOR CHILDREN & ADOLESCENTS: (up to 17 yrs.) Home based Services for Children and Adolescents provides for those children and adolescents who are at risk of placement outside of the home. Our clinicians work closely with parents, schools, and community agencies to maintain the children or adolescent in the community. This service will provide a home environment for those children who cannot stay in their own home setting. OTHER SERVICES: EMPLOYEE ASSISTANCE PROGRAM: Four County has an Employee Assistance Program. This is a business tool to help employers maintain a healthy, safe environment and a productive work force. TESTING AND EVALUATION: We offer psychological testing, psychiatric evaluations, and medication clinic. In addition, we perform disability evaluations and addictions evaluations. STAFF: Four County Counseling Center has over 80 clinical staff members. This includes three full-time psychiatrists, psychologists, advanced nurse practitioners, Registered Nurses, Social workers, and Case managers. In addition, a large support staff to assist with patient care. FAMILY SUPPORT SERVICES: For those individuals that need childcare while receiving therapy, Four County Counseling Center has contracted with Area V and the Head Start program for this service. Country View motel has agreed to house these individuals who are in need of housing. For consumers who are in need of primary medical/obstetrical care, we have contracted with Logan Internists Inc. to provide those services. We refer to the Community Health Clinic for all other needs. Four County staff provides transportation. Immunizations are monitored by the Community clinic and internally. All children and adolescents are requires at intake to complete our immunization screen. If there are needed immunizations the Community Health Nurse is contacted. All services address developmental issues through case management and staff age specific competency training. All Four County Staff are alert and trained to assess for sexual and physical abuse. Policy and Procedures are in place for reporting these incidents. ADMINISTRATION OF SERVICES: Four County Counseling Center oversees the continuum of care process with case managers who work in treatment teams to assist the consumers with the smooth transition of services. This is accomplished through a holistic approach with input from the consumer, psychiatrist, and clinicians. Each consumer has, upon entry into Four County Counseling Center, a case manager assigned to them. All rights and responsibilities are reviewed with the consumer at this time and consent for treatment is signed. Along with the treatment team, the consumer assists in developing an individualized treatment plan to address identified needs and to increase the consumer’s coping skills and control of symptoms. Initially an Intake is completed and a treatment plan developed. Strengths and weaknesses as well as barriers to learning are identified. Individual goals and objectives are agreed upon and are reviewed regularly. Family or significant other involvement is encouraged. Coordination of services- including contracted services and community resources- is accomplished through the continuum of care by the case manager to assure seamless entry into the various services that are appropriate to meet the consumers’ needs and enable them to reach their optimal level of wellness. The case manager’s responsibility is to assess the needs of the consumer not only in relationship to therapy, but all factors that impact effective outcomes such as transportation, childcare, housing, community resources, etc. Every effort is made to attain the least restrictive level of care, however, on occasion a crisis develops. Crisis intervention is assured via Emergency Services and through a case manager on-call beeper system. All consumers who are admitted to the Inpatient Acute Care Unit have a concurrent review completed within 24 hours of admission to ascertain appropriateness of entry into this restrictive level of care in our continuum. Risk Management and Performance Quality Improvement reviews medication usage, consents, side effects, adverse drug reactions and appropriateness of the medications. Monthly monitors are completed by registered nurses. Our Pharmacy and Therapeutics Committee meets quarterly. Audits of prescribed medications are completed by the pharmacy in relationship to Four County Counseling Center’s formulary. Peer Reviews of all clinical departments are completed monthly to assure quality, appropriateness and continuity of care. To assure appropriate utilization of services, the staff psychiatrist along with the treatment Team reviews the Treatment Plan periodically. Utilization management and overview is accomplished in the Utilization Review committee (URC) and through Performance quality Improvement monitors. A systematic process is in place to review consumers whether they are within our center or in a contracted environment. The URC reviews those services that are outliers based on various parameters. For those consumers who receive contracted services outside Four County Counseling Center, the URC may request any consumer’s chart for review. The case manager’s are routinely in contact with the contracted provider’s staff. All contracted providers have a utilization management process within their facility also. Treatment team meetings are used to monitor progress and use of services provided. Satisfaction surveys are also utilized as part of an overarching process of oversight and management. |
